Quality Nursing, Speech Pathology, Physical Therapy, and Occupational Therapy services are available. The professional staff has advanced training to provide the skilled medical and emotional support that you need to maintain independence in your home.
Indications for home health care include recent hospitalization, a change in medical condition or change of functional ability, history of falls or unsteady walking and balance issues, and new or longterm illness or injury.
Referrals may be made by a rehabilitation center, hospital, social worker, physician, family member, or friend. Self referrals are also accepted. Once the referral is made, the patient’s physician will be contacted for orders and a plan for services will be developed with physician oversight.
